Aftercare delivers comprehensive, long-term stabilization for individuals with the highest level of need. The program is performing within expected benchmarks while taking on higher-acuity work that drives deeper impact. Aftercare is not simply outreach—it is a system-level intervention addressing the root causes of emergency service utilization.
Aftercare prioritizes depth of engagement over volume, which is essential for working with high-acuity clients.
This approach allows the program to:
- Build trust with individuals who are often difficult to engage;
- Address multiple barriers at once (housing, health, income, etc);
- Prevent repeated crisis cycles; and
- Support long-term stabilization rather than short-term fixes.

Community Interactions
Healthcare Access & Transportation
Aftercare supported a client in enrolling with Ride Connection, a transportation service for individuals on Medicare, enabling attendance at a long-overdue denture appointment after a 5-year gap in oral health care. Obtaining dentures supports improved self-esteem and increased social engagement, highlighting how addressing both medical and psychosocial needs can create meaningful, life-changing outcomes.
Technology & Connection
Recognizing the importance of technology for social, educational, and vocational engagement, the Aftercare team assisted two clients in obtaining mobile phones. Having access to technology helps individuals stay connected to services, community, and support networks. This intervention increased access to essential resources, strengthened community connections, and supported greater independence and social integration.
Behavioral Health & Recovery Support
A client with a history of Substance Use Disorder (SUD) and challenges maintaining sobriety initially engaged in outpatient treatment but experienced barriers related to past trauma and reliance on unhealthy coping strategies. The Aftercare team supported the client in accessing Hooper Detox, providing one-to-one support during the intake process and remaining on site to assist both the client and staff, given the client's history of agitation in high-stress situations.
During intake, the client became too agitated to continue and began experiencing a mental health crisis. Aftercare supported the client in exploring options in the moment. Due to the trust built with the team, the client accepted support, and we were able to facilitate transport to Unity for stabilization. The client remained motivated to pursue detox following stabilization. Aftercare coordinated with Unity staff to ensure a smooth transition back to Hooper Detox once the client was ready to complete intake.
After successfully completing detox, the client transitioned to the 16th & Burnside Residential Program, a 90-day treatment program. This coordinated, trauma-informed approach ensured the client remained engaged in services despite crisis, resulting in successful entry into long-term treatment and increased opportunity for sustained recovery and stability.
Crisis Response & Stabilization
Following a new client referral, Aftercare found the individual in a non-responsive state and experiencing communication challenges, making it difficult to engage. The team connected with the client and their partner, talking through what was happening and sharing options for support, including stabilization services and next steps for care.
Aftercare supported the client and their partner with admission to Unity for stabilization. Over the following 2 weeks, the client showed improved communication, increased engagement, and shared appreciation for the support, reflecting the impact of timely outreach and connection to care.
Long-Term Housing Stability
Aftercare supported a client over the past 4 years with stabilization services, including housing navigation, resulting in the client successfully obtaining permanent housing through New Narrative. After move-in, Aftercare will continue short-term support (1–3 months) to help the client connect to services and adjust to housing, supporting ongoing stability.
Lasting Impact & Trust
A former client reached out and shared that past outreach and support from Aftercare helped them get into stable housing. This highlights the program's role in supporting long-term stability, the trust built through engagement, and the client's growing sense of pride in maintaining that stability.
Housing Placement & Safety
Aftercare is working with a client who had been unhoused for 8 months after relocating to Portland. They had been taken advantage of and were a victim of violence while on the streets. They experienced multiple medical concerns. Aftercare supported the client through the housing process, including conducting an assessment from Multnomah County's Homeless Services Department. The client was accepted into and successfully moved into Bud Clark Commons, providing a safe and supportive environment to begin addressing their health and ongoing needs. Aftercare will continue follow-up support after move-in to help ensure the client is connected to services, both within Bud Clark Commons and in the broader community, supporting ongoing stability.
Housing Navigation & Advocacy
Two clients have been placed on the priority list for Single Room Occupancy (SRO) housing following completion of their assessment from Multnomah County's Homeless Services Department. Aftercare continues to provide ongoing support and engagement with both clients while they await housing placement. Community Health Workers regularly attend monthly collaborative meetings to receive updates on the county's housing assessment list and advocate for client progress.
Healthcare Coverage Access
One of Aftercare's Community Health Workers supported a client through the application process and successfully connected them to the Oregon Health Plan, improving access to needed healthcare services.
