Aftercare

The Aftercare program is part of Portland Street Response and is staffed by Community Health Workers and Peer Specialists. Aftercare provides follow-up support to people after an initial crisis response, when they are interested in connecting to services.

About Aftercare

Our team builds relationships, advocates alongside individuals, and helps connect people to medical and behavioral health care, housing and shelter resources, recovery and substance use services, and benefits such as the Oregon Health Plan. Through ongoing outreach, advocacy, and coordination, we support people in navigating complex systems and accessing care that can be difficult to reach without support.

By staying engaged over time, Aftercare helps reduce repeat crises, improve connections to community-based services, and support long-term stability and well-being. Aftercare is not a direct referral program for the public or outside agencies; services begin through Portland Street Response following a crisis call.
