General Liability - Risk Management

Claims delivered in-person or by mail, will be recorded on the date received by City Risk. Claims faxed or e-mailed and received before the close of business, will be recorded as received on that date. All claims delivered after business hours will be recorded as received on the next business day.

City Risk hours are Monday through Friday, 8 a.m. to 5 p.m. 

City Risk is closed for official holidays. 

Required forms for filing a claim are listed below:

For personal injury or property damage not involving a City-owned vehicle

for personal injury or property damage involving a City-owned vehicle.

for damages related to Aircraft, Crime, Cyber Liability, Fine Art, Heliport, Portland Fire and Rescue Boats, Portland International Raceway, Pioneer Garage, Portland Aerial Tram and other property.
