Amusement devices permit application

Service
Any business that offers games which cost money to play must apply for a permit and pay a fee. A permit is required whether or not the business owns the devices. Permits expire on December 31 and must be renewed each year.

This form should take no longer than 10 minutes to complete

  1. Count your qualifying devices

    Regulated devices include:

    • Pool tables
    • Foosball
    • Coin operated darts
    • Claw or grabber cranes
    • Trivia games
    • Pinball machines
    • Shuffle board
    • All video games (except state-owned video poker terminals)

    Do not include:

    • Jukeboxes
    • Vending machines
    • State-owned video poker
    • Keno
    • Pull tab or lottery devices
