Annual Reporting Forms

Affirmative Fair Housing Marketing Plan (AFHMP Form)   (726 Kb) – HUD.gov

Download and save the AFMHP form to your computer. The form can be filled out by tabbing through each field. Required reporting is every five years. If updating is required, the portal will identify this requirement. 

If the Owner or Agent has determined that, based on a review of the data in the previously approved AFHMP that no revision is needed, PHB will accept this acknowledgement printed on company letterhead indicating that an updated AFHMP is not necessary.

Forms for Determining Eligibility

The TIC is the tool you will use to calculate income and determine tenant eligibility for programs like HOME and PHB-funded programs. Be sure to use the correct TIC.

Certain projects that qualify as facilities may request an exemption from tenant income reporting by submitting this questionnaire.

PHB requires property inspections at least once every three years, but inspections may be conducted more often. If no inspections from other funders are held in this time frame, a self-inspection must be completed and submitted with your annual reporting submission every three years.
