Verification Documents

PHB staff will provide guidance when applicants reach the verification stage. See examples of types of verification documents used for the N/NE Preference Policy Waitlist.

PHB may require a waitlist applicant to provide up to three verification documents, one from each list: A, B, and C. Documents must clearly show name and address.

List A – Applicant Identification

One type of photo ID from List A:

  • Photo ID
  • Passport
  • Foreign Passport
  • Permanent resident card
  • Green card
  • Military identification cards

List B – Applicant Proof of Address

  • Bank documents
  • Court documents
  • Credit card documents
  • Credit reports
  • Current/Previous driver’s license
  • Household bills (lights, water, gas, electric bills, etc.)
  • Income tax records
  • Insurance documents (Car, life, renters, etc.)
  • Lease agreements
  • Marriage licenses
  • Membership registries
  • Military identification cards
  • Mortgage documents
  • Phone bills
  • Posted mail
