Jurisdictions that use these forms

These local request forms are used to request an environmental review to access HUD funding for a project. The forms may be used by agencies in the following jurisdictions:

  • Multnomah County
  • City of Portland
  • City of Gresham

When to submit a request

To be able to access grant funding in time, how soon in advance should an agency request an environmental review?

  • Homebuyer and other Tier 2 programs: 7 days
  • Multi-family: 120 days

Environmental Review Request Forms

Select a form for the project type. Once submitted, the Certifying Officer will contact you to confirm the review process and timeline.

Homebuyer

In the application:

  • Name of homebuyer
  • Address of homebuyer
  • Grant name
  • Grant amount
  • Full price of home

Required attachments:

  • FEMA Firmette
  • If home is in floodplain, copy of flood insurance requirement
  • Google map showing distance between address and PDX airport

Multi-family

Includes homebuyer condo projects and rental housing projects.

In the application:

  • Name, title, contact of preparer
  • Name of agency (e.g., Habitat, Home Forward)
  • Address of project
  • Target date for ground breaking or other “choice limiting action”
