Jurisdictions that use these forms
These local request forms are used to request an environmental review to access HUD funding for a project. The forms may be used by agencies in the following jurisdictions:
- Multnomah County
- City of Portland
- City of Gresham

When to submit a request
To access grant funding on time, agencies should submit their request with enough time for processing.
- Homebuyer and other Tier 2 programs: 7 days
- Multi-family: 120 days
What to expect in an Environmental Review
- Plan ahead with enough time for the request to be processed. Multi-family projects require more time than Tier 2 reviews.

- Requests for consultation with other governmental entities, including documentation needed for that process, such as:
- Fish and Wildlife Service
- NOAA National Marine and Fisheries Service, West Coast region
- Native American tribal governments in Oregon
- State Historic Preservation Office

Homebuyer
In the application:
- Name of homebuyer
- Address of home to be purchased
- Grant name
- Grant amount
- Full price of home
Required attachments:
- FEMA Firmette
- If home is in floodplain, copy of flood insurance requirement
- Google map showing distance between address and PDX airport

Multi-family
Includes homebuyer condo projects and rental housing projects.
In the application:
- Name, title, contact of preparer
- Name of agency (e.g., Habitat, Home Forward)
- Address of project
- Target date for ground breaking or other “choice limiting action”
