Report PP&R Incident

This page is for internal PP&R staff to report security, safety, property damage, and bias-hate incidents in parks. The public needing to report park incidents should call Portland Park Rangers at 503-823-1637.

Three steps for PP&R staff to report an incident in parks:

  1. For emergencies, always call 911.
  2. For park rule violations, call Portland Park Rangers at 503-823-1637.
  3. In all cases, close the loop by filling out the PP&R Incident Report form below. 

Once your form is submitted, you may be asked by the Safety team to complete additional forms for workers compensation, guest accident, or property damage. Refer to the PP&R Red Book for more information.

This form is for Portland Parks & Recreation internal use only. Unauthorized submissions will be disregarded.

Please Note

For staff or visitor injuries that result in a trip to a hospital or clinic, contact the safety team at Park Ranger Dispatch 503-823-1637.

  • For guest accidents and employee injuries, additional forms are needed. Please refer to the Red Book.
