Refer Someone to Ceasefire

Ceasefire works with individuals who may be at the highest risk of involvement in gun violence. Through outreach, services, and community partnerships, we connect individuals to support and opportunities that help prevent violence and promote safer pathways.

If you believe someone may be at high risk of involvement in gun violence, you can submit a referral. Referrals may be submitted by community members, family members, service providers, or partners.

All referrals are reviewed by the Ceasefire team and coordinated with trusted community partners to determine the most appropriate outreach and support.

Who Should Be Referred

Individuals who may:

• Be at high risk of involvement in gun violence
• Have been recently involved in a gun violence incident
• Be experiencing ongoing conflict that could escalate to violence
• Benefit from outreach, mentorship, and support services

The Ceasefire Community Response Program is committed to handling all submitted information with care and discretion. Information received through this form will be shared only with City staff and contracted partners directly involved in assessing eligibility and coordinating services. 

Please be aware that as a City program, submissions are subject to the Oregon Public Records Law, which generally presumes public records are available for disclosure upon request. The City will assert all applicable exemptions to protect sensitive information where the law allows, but cannot guarantee that submitted information will remain private in all circumstances. Records may also be subject to disclosure by court order or subpoena. 

If you are submitting a referral on behalf of someone without their knowledge, please be aware that your identity and the information you provide may not remain confidential in all circumstances. If you have concerns about privacy before submitting, please contact the Ceasefire team directly at ceasefire@portlandoregon.gov.
