Updated Deferred Comp Forms Help You Plan Ahead

Deferred Compensation forms for the 3-Year Catch-Up and Final Paycheck have been updated and are now available. Employees are encouraged to email completed forms to Benefits@portlandoregon.gov at least five (5) days before their retirement date to ensure timely processing.

Two Deferred Compensation forms have been updated and are now available:

  • 3-Year Catch-Up Form
    Used by employees who are nearing retirement and want to increase their deferred compensation contributions above the standard annual limit, if eligible. This option allows you to "catch up" on unused contribution limits from prior years.
  • Final Paycheck Form
    Used by employees who want to make a deferred compensation contribution from their final paycheck when separating or retiring from the City.

Paper forms are accepted; however, to ensure timely processing, we strongly encourage emailing your completed form to Benefits@portlandoregon.gov.

Important: Forms must be received at least five (5) days before your retirement date to meet the deadline to participate. Employees who have already completed forms do not need to send new ones our way.
