Gather Request and Authorization
In accordance with the Oregon State Law, BHR is only able to provide the following information for employment verification: Dates of employment and position held.
Please note that if you are requesting additional information on behalf of a current or former City employee, you will need a completed authorization from the employee.
You will need to provide us:

- Employee Authorization
- Employee Full Name
- Desired Return Date

Employee Verification: 3rd party requestors such as a lender or potential landlord may request employee information such as,
- Active or Inactive status
- Job Title/Classification
- Employment Dates
- Year to date and previous two years wages
Employee Reference: 3rd party requestor, such as a new employer or education institution, may request employee information such as:
- Tardiness
- Attitude
- Skills or assets
Employee Records Request: An employee's full record, including the following information:
- Performance evaluations records
- Disciplinary actions records
- Employment history records
- Driving records
City employees do not need to submit a release if information for to request their record.
Non-City or 3rd parties must complete the City’s ROI form: City of Portland Release of Information.
Letter of Employment: A letter on BHR letterhead confirming active/inactive status, job title/classification, hourly wage, employment dates.

- Employee should login to Home | Federal Student Aid and fill out either a paper or electronic form and send to BHRvoe@portlandoregon.gov

Who can submit an employment verification, employee reference, or Employee Personnel Records?
- Either the employee or 3rd Party such as mortgage lenders, tenant screening services, or other related services can initiate a request.
- Employee Personnel Records can be sent to the BHRvoe@portlandoregon.gov or directly to Jen Casillas: jennifer.casillas@portlandoregon.gov
What verification information is needed to submit a request?
- Employee must verify DOB or the last four digits of their SSN only.
- 3rd Party requests must provide a signed release of information or City of Portland Release of Information form
