Insurance requirements for sewer access authorizations

Every request for authorization to access the sewer and/or stormwater systems will require a certificate of liability insurance, a certificate of workers’ compensation insurance, and related endorsements. Learn more on this page.

Access Authorization Insurance Requirements

Your project is required to have a certificate of liability insurance with an endorsement on file with the Bureau of Environmental Services for the duration of your Access Authorization. This document will be submitted with your application for review. Do not submit your insurance separately or ahead of your access request. The certificate and endorsement must meet the requirements detailed below and others, if updated at the time of your work request. The certificate and endorsement must: 

  • Be on file and approved prior to issuance of your authorization(s), 
  • Be replaced prior to expiring, and 
  • Remain in effect for the duration of the authorized work. A lapse in coverage or failure to renew coverage can result in the stopping of authorized work or revocation of your authorization(s).

This information should be provided directly to your insurance agent to aid them in issuing a certificate of liability insurance and endorsement that complies with the City of Portland's insurance requirements for the Bureau of Environmental Services. Certification and Endorsement Requirements

Applicant must provide a certificate of liability insurance, a certificate of workers' compensation insurance, and related endorsements. These items must meet the requirements below.

Certificate of Workers' Compensation Insurance

  • Workers' Compensation coverage on a statutory basis for all persons employed in performing services, in accordance with ORS Chapter 656, either as:
    1. a carrier-insured employer; or
    2. a self-insured employer.
  • Coverage shall include Employer's Liability Insurance with coverage limits of not less than $100,000 for each accident, a $500,000 disease "policy" limit, and $100,000 disease "each employee" limit. 

Certificate of Liability Insurance

  • Commercial General Liability coverage of a minimum of $1,000,000 (one million dollars) per occurrence; $1,000,000 (one million dollars) Personal Injury/Advertising Injury; and $2,000,000 (two million dollars) General Aggregate
  • Automobile Liability coverage of a minimum $1,000,000 (one million dollars) combined single limit per occurrence, covering owned, non-owned and hired vehicles
  • Pollution Liability Insurance coverage for bodily injury, property damage, and environmental damage resulting from sudden accidental pollution and related cleanup costs. Coverage must be for a minimum of $1,000,000 (one million dollars) per occurrence and $3,000,000 (three million dollars) General Aggregate.
  • City of Portland Bureau of Environmental Services named as the certificate holder using the address:

  • The description of operations section of the certificate must include the following note, specific to your project: All operations of the named insured, including all operations by subcontractors, performed under an Access Authorization provided by the City of Portland for Bureau of Environmental Services

Endorsement

  • CG 20 12 endorsement or an equivalent endorsement
  • Names the "City of Portland Bureau of Environmental Services" if the form names the additional insured entity
