This request is for a property subject to tiered stormwater billing. The property is occupied by a residential single-family home, is associated with a single City of Portland stormwater utility account, does not contain any other dwelling units, and is within the Portland city limits.

I acknowledge that all the information contained within this form is both accurate and complete and that the stormwater billable area segments listed in this form significantly represent current stormwater billable areas as described in Portland City Code Section 17.36.050 and Administrative Rule ENB-4.09.

I understand that, by submitting this form, data may be transferred to the City of Portland Bureau of Environmental Services who, at their discretion and utilizing methodology they deem appropriate, may or may not update records that relate to or could influence the stormwater and related charges associated with the property address I have referenced in the form.

I understand that, in accordance with Portland City Code and administrative rules, the City of Portland Bureau of Environmental Services will make the final determination regarding the measured or estimated area of all property segments comprising stormwater billable area and that that determination is not appealable.

Any change in stormwater billing resulting from data provided to the City will be effective as of the date the City reviews and confirms the accuracy of all information contained in this form. Changes to stormwater billing may not, under any circumstance, be retroactive.

Data in this form may be reviewed or validated prior to transferring information to the City of Portland Bureau of Environmental Services for review of stormwater billable area. If the City is unable to confirm or validate the information you provided, or if the City determines this request is not appropriate, the City may refuse to transfer your request to the City of Portland Bureau of Environmental Services for a stormwater billable area review and may issue you a written or electronic notification informing you of this determination.
